My change proposal is being implemented in the ICU. The intervention is implementing communication tools and processes that are evidence based to improve nursing sensitive indicators in the ICU. The internal stakeholders are the ICU staff and the hospital. The external stakeholders are the community that is served by the hospital. Our hospital works off of a relationship-based care (RBC) model. RBC is a culture transformation model and an operational framework that improves safety, quality, patient satisfaction, and staff satisfaction by improving every relationship within an organization (Gallison & Kester, 2018). The core of workforce engagement is the reignighting of joy and meaning for nurses. The joy and satisfaction in having a sense of accomplishment and significance in the work through processes leading to successful outcomes. RBC speaks to how we treat patients, family, and each other. Internal stakeholders all work off this model in this organization. However, covid greatly challenged relationship based care principles by limiting how we interact with each other and our patient families. That in person piece is missing for many patients still. In this organizational transition back to pre-covid practices, meetings, and policies staff are looking for guidance to unify and strengthen the workforce. It is a good segway into external stakeholders. Our nurses and other staff are also members of the community served by the hospital. Therefore, the internal stakeholders all face the real fact that they too receive their care here and have an interest in the quality of care provided. This community funded hospital has been influenced by local donors, architects and artists. Donors play a large role in celebrating the staff and creating this sense of meaning and significance for hospital staff. In a relationship based care model, these gestures serve a huge purpose and allow the hospital to recognize staff in unique ways. The positive factor here is that the nurses have come out of this powerless feeling covid left them with. Small gestures that build trust between nursing and management create a more productive work environment. This is done through clear concise communication, open discussion, and acting on feedback from staff.

Stakeholder involvement is crucial for the successful implementation of the change proposal project. Stakeholders are those who are interested in the change proposal project, such as nurses, patients, and suppliers. They can affect or be affected by the organization’s actions, objectives, and policies (Lubbeke et al., 2019). We must assess our work environment to identify all relevant stakeholders. Failure to do so can negatively affect the project. 

It is essential to measure how influential each stakeholder is. The more influential the stakeholder, the more support we want from them. Having a strategy to enhance stakeholder support is of the essence. It is vital to ask for the input of our stakeholders and keep informed of the process and progress of our project. We must ask ourselves: what are their goals? Ideas? Expectations? Stakeholder involvement can vary in intensity and has been described as occurring on a continuum, from passive to active (Nilsen et al., 2020). Passive stakeholders are individuals whom we keep informed of our project, while we also have actively involved stakeholders. There can be various stakeholders, and to work with them all, we should identify their relationships and remain engaged with them all, which can be challenging. My change project focuses on reducing healthcare-acquired infections, where patients, healthcare workers, the organization, and the community are essential stakeholders.

Stakeholders are important in support because they can help with resources, encouragement, and power in leadership to implement change. Having Stakeholders engagement can help with motivation in other employees, budgeting, trust-building, job satisfaction, and developing intangible resources (Mitchell et al, 2022). For my proposal, my boss and the nurse practitioner are the stakeholders. They are the key for if change happens in our office, if they approve, then it happens. If not, then change cannot be implemented. I would need their support for the change in educating the multiple sclerosis patients on proper nutrition. The providers in the office are who implement change in how the office functions, how and what practices are done. In order, to secure their support I would provide then with the data collected and encourage that I would be there to support the patients as well in the nutritional adventure. I already have great rapport with the nurse practitioners, so I don’t see the proposal hitting any brick walls.

Technology is an important part of my implementation for my capstone project. As the aging population and life expectancy increases, and issues arise such as nursing staffing shortages, these new medical technologies are crucial for continued patient care, and the healthcare system. “The implementation of Electronic Health Record (EHR) systems is meant to assist providers’ evidence-based decision making and streamline providers’ workflow via efficient coordination for patient care” (Upadhyay & Hu, 2022). Therefore, the information documented in the patient’s electronic healthcare record can be accessed by all healthcare professionals involved in the patient’s care and by the patient as well. Electronic health records are like a checklist for nurses to help ensure and monitor that all the necessary steps are taken to ensure the patient reaches their healthcare outcome goals and help nurses decrease mistakes and save time by reducing nursing burnout and stressors. Technology changes may help cover the impacts of innovative changes on products procedures, and channel distribution processes.

Technology is super beneficial when implementing new projects. The one piece of technology I plan on having nurses utilizing more frequently is the patient lift device. This device is utilized to transfer patients from one position to another and helps reduce the risk of harm for both nurses and patients (FDA, 2018). This device is great, as it will allow nurses to easily ambulate patients from the bed to the chair if they are unable to aid in ambulation. This device is stored on the Medical Surgical Floor but is rarely used due to lack of education. This should not be the case, as every single nurse at the hospital is required to take an education class on lift use yearly. However, when you do not use the education, you lose it. Thus, I will reach out to the safety department and ask for a unit educational class on the use of this lift. I believe that more patients will be ambulating with the use of this technology and less nurses will be hurting themselves trying to move heavier patients.

Since the original Institute of Medicine (IOM) report was published, there has been an accelerated development and adoption of health information technology with varying degrees of evidence about the impact of health information technology on patient safety (Alotaibi & Federico, 2017). Patient safety is a subset of healthcare and is defined as avoiding, preventing, and ameliorating adverse outcomes or injuries stemming from healthcare processes (Alotaibi & Federico, 2017). Sign-out or “handover” communication relates to passing patient-specific information from one caregiver to another, from one team of caregivers to the next, or from caregivers to the patient and family to ensure patient care continuity and safety. Breakdown in the handover of patient information is one of the leading root causes of sentinel events in the United States (Alotaibi & Federico, 2017).

An EPIC software is a very effective medical record and charting tool in our workplace. We have an electronically hand over communication feature, a handy tool our nurses tend not to use. They have a handwritten shift report sheet to pass for every shift change. If nurses must chart in hand over on epic what education they gave, the outcome, and what we need to address and emphasize more frequently. Nurses also manage hand-out reports that which is the way fall prevention education is more effective for patients and their families. Computers store data better than paper trails to see past information about patients.
