Statement of Purpose and Objectives *

Our statement is intended to give you the opportunity to submit a tailored narrative that will describe your education, experience and professional career objectives. There are no length or format requirements.

It should include the following points:

· why you are interested in pursuing a public health degree

· why you have chosen this concentration

· how you became interested in this program at Florida International University, and your career goals.

The program is MSW Master of Social Work my work history in the field is listed below:

Office Manager/Service Coordinator

· Complete assessments with member/provider to obtain information regarding client status, support system, and need for services for care plan development

· Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care

· Manage, monitor and revised up to 100 client service plans to meet member’s needs of optimizing long term therapy services and support needs

· Serve as a mentor to new staff

· Collaborate with providers, discharge planners, Therapist, and other parties to ensure appropriate therapy plan.

Case Manager

· Managed 60-70 student case files; actively sought ways to help students

· Planned and delivered student support services including tutoring, college and career exploration programs, health awareness, guest speaker series, and community service projects

· Delivered leadership and youth development activities with varied curriculums

· Coordinated all school-based activities in collaboration with principals, teachers, and staff. Including selecting dates and students, planning logistical details, and complying with District forms and protocols

· Provided individual counseling and referrals to students at risk of dropping out which include low income and academically disadvantaged families.
